Sacramento has plenty of surgeons of its own, so this page is specific about the drive’s purpose: a breast lift alone or with implants, implant removal and revision, and reconstruction after cancer. A breast lift raises and reshapes the breast. It removes excess skin, moves the nipple-areolar complex higher and can reduce enlarged areolas. It suits breasts that have dropped but still have the volume you want.
When volume has gone too, usually after pregnancy, breastfeeding or weight change, an implant alone can make the drop more obvious. Augmentation-mastopexy does both in one session under general anesthesia. It takes about three to four hours, and you’ll need about 6 weeks before returning to unrestricted activity. You and Dr. Malotky decide on placement above or beneath the muscle based on your tissue and frame.
If you have implants you no longer want, removal can be paired with a lift. Dr. Malotky may recommend a lift after removal to improve the shape.
For breast reconstruction, Sacramento patients get a plan built around their cancer treatment. Reconstruction after mastectomy or lumpectomy can use implants, your own tissue or both. It can be done right away or in stages. Radiation doesn’t rule it out, but it can change the timing and method, and tissue-based options are often considered.
For implant problems, en bloc capsulectomy removes the implant and its scar capsule in one piece. It’s often chosen for rupture, capsular contracture or suspected breast implant illness. A deflated saline implant can come out through implant removal, done in the office under local anesthesia. Charlotte had lived with hard, encapsulated implants for 40 years: “Dr. Malotky removed my implants and capsules surrounding my implants, along with a breast lift to create my new natural breasts.”

Augmentation-mastopexy and en bloc capsulectomy are done in the operating room under general anesthesia. Implant deflation and removal is an in-office procedure under local anesthesia, and most patients are back to light activity within a few days.
Staged reconstruction means several visits. Later stages are scheduled around your recovery and preference, and follow-up appointments continue between stages. At about 2 hr 25 min each way, it’s worth mapping those visits out early.
